Background:

Palmoplantar psoriasis is a debilitating variant of psoriasis affecting the palms and soles.
It significantly impacts patient quality of life and mental health, often proving resistant to conventional therapies.

Observation:

A male patient with a 15-year history of psoriasis and severe palmoplantar involvement showed minimal response to topical, acitretin, and adalimumab treatments.
The patient's condition was refractory to multiple established treatment modalities.

Findings:

Treatment with risankizumab, a selective anti-interleukin-23 (IL-23) antibody, led to complete resolution of palmoplantar lesions.
Complete clinical remission was achieved after the fourth dose of risankizumab.

Implications:

Risankizumab demonstrates significant efficacy in managing treatment-resistant palmoplantar psoriasis.
This suggests a potential new therapeutic avenue for patients with severe palmoplantar psoriasis unresponsive to other treatments.
