Insights

Early changes in lymphocyte counts can predict COVID-19 severity. Patients with low lymphocyte counts (LYM) on admission and a further decrease within 72 hours are at higher risk for severe coronavirus disease 2019 (COVID-19).

Area of Science:

  • Infectious Diseases
  • Immunology
  • Clinical Medicine

Background:

  • Coronavirus disease 2019 (COVID-19) is a global health concern.
  • Lymphocyte count (LYM) is a key indicator in immune response.
  • Early dynamic changes in LYM may correlate with disease progression.

Purpose of the Study:

  • To investigate the relationship between early dynamic changes in lymphocyte counts and COVID-19 severity.
  • To identify specific lymphocyte patterns predictive of severe or critical illness within 72 hours of admission.

Main Methods:

  • Retrospective study of 104 COVID-19 patients.
  • Patients categorized into four groups based on initial lymphocyte count (normal or lymphopenia) and change within 72 hours (ΔLYM).
  • Cox regression and Kaplan-Meier survival analysis used to assess the probability of developing severe/critical illness.

Main Results:

  • 20.2% of patients developed severe/critical COVID-19.
  • Patients with initial lymphopenia and a subsequent decrease in LYM (Group 4) had a significantly higher risk (HR=6.4) of severe/critical illness compared to those with normal LYM and no decrease (Group 1).
  • Age, C-reactive protein (CRP), and albumin (ALB) were also factors in multivariate analysis.

Conclusions:

  • Early dynamic changes in lymphocyte counts are significantly associated with COVID-19 severity.
  • Patients exhibiting lymphopenia on admission with a continued decline in LYM within 72 hours face a substantially elevated risk of progressing to severe or critical disease.
