    Summary

Catamenial pneumothorax, a rare form of pulmonary endometriosis, was diagnosed primarily through clinical history in a patient with limited other endometriosis evidence. Surgical pleural ablation was chosen over prolonged medical therapy to prevent recurrent, life-threatening pneumothoraces.

    Area of Science:

    • Reproductive Medicine
    • Pulmonology
    • Surgical Pathology

    Background:

    • Catamenial pneumothorax is a rare manifestation of endometriosis, characterized by spontaneous collapsed lung occurring in relation to menstruation.
    • Pulmonary endometriosis, while uncommon, presents unique diagnostic and therapeutic challenges, often requiring a multidisciplinary approach.

    Observation:

    • A case study focusing on a patient with catamenial pneumothorax where clinical history was paramount for diagnosis, despite minimal evidence of endometriosis elsewhere.
    • The patient's age and parity influenced the decision against prolonged medical management.

    Findings:

    • Surgical pleural ablation was performed to effectively manage and prevent recurrent episodes of life-threatening pneumothoraces.
    • Diagnostic investigations useful for such cases are reviewed, and the patient's clinical profile is compared with existing literature.

    Implications:

    • Highlights the importance of clinical suspicion in diagnosing rare conditions like catamenial pneumothorax.
    • Demonstrates the efficacy of surgical intervention in managing recurrent catamenial pneumothorax, particularly in select patient populations.
    • Contributes to the limited case reports, aiding in understanding the clinical spectrum and management of pulmonary endometriosis.
