Background:

  • Chikungunya virus (CHIKV) is an arbovirus causing outbreaks, with neurological complications being a significant concern.
  • Myeloneuropathy is a rare, potentially immune-mediated neurological complication of CHIKV infection.

Observation:

  • A 45-year-old male presented with acute quadriparesis, respiratory distress, and sensory deficits 6 weeks post-CHIKV infection.
  • Clinical examination revealed flaccid quadriparesis with areflexia and a distinct sensory level.
  • MRI showed extensive spinal cord hyperintensities and muscle signal changes, with normal brain imaging.

Findings:

  • Positive IgM CHIKV antibodies confirmed recent infection.
  • The patient received plasmapheresis and corticosteroids, leading to partial improvement in upper limb function but persistent paraparesis.

Implications:

  • This case highlights unusual MRI findings in CHIKV-associated myeloneuropathy.
  • Early recognition and prompt immunomodulatory treatment may improve outcomes for post-infective neurological complications.
