Summary

Adjuvant radiotherapy (ART) significantly improves outcomes for retroperitoneal sarcoma (RPS) patients, enhancing overall survival and reducing recurrence. Adjuvant chemotherapy (ACT) showed no benefit and potentially worse survival in RPS management.

Purpose of the Study:

  • To assess the effectiveness of adjuvant therapy in managing retroperitoneal sarcoma (RPS) patients.
  • To compare outcomes of adjuvant radiotherapy (ART) and adjuvant chemotherapy (ACT) against surgery alone.
  • To provide evidence-based insights for treatment recommendations.

Main Methods:

  • A systematic literature search was conducted across major databases (PubMed, Web of Science, Embase, ASCO, Cochrane) until December 2020.
  • Comparative studies of adjuvant therapy versus surgery alone were included.
  • Meta-analysis evaluated overall survival (OS), local recurrence (LR), recurrence-free survival (RFS), and metastasis-free survival (MFS), summarizing data as hazard ratios (HR) with 95% confidence intervals (CI).

Main Results:

  • Fifteen eligible trials included 9281 adjuvant therapy cases and 21,583 surgery-alone cases.
  • ART demonstrated significant benefits: longer OS (HR=0.80), longer RFS (HR=0.61), and lower LR (HR=0.31) compared to surgery alone.
  • ACT did not show significant benefits for OS, RFS, or MFS, and sensitivity analysis indicated a worse OS (HR=1.19) with ACT.

Conclusions:

  • The quality of evidence for most outcomes was moderate.
  • Adjuvant radiotherapy (ART) is supported by evidence to generally achieve better outcomes than surgery alone for RPS.
  • Adjuvant chemotherapy (ACT) does not appear to benefit RPS patients and may be detrimental.
