Background:

  • Primary peripheral T-cell central nervous system lymphoma (PCNSL) is a rare, aggressive tumor.
  • PCNSL presents a diagnostic challenge due to its mimicry of other neurological conditions.
  • Immunocompromised individuals and those with autoimmune conditions have an increased risk.

Observation:

  • A 61-year-old woman with Sjögren's syndrome presented with progressive neurological deficits.
  • Radiographic imaging revealed an enhancing, infiltrative left frontal lobe lesion with vasogenic edema.
  • The lesion was initially suspected to be a primary or metastatic neoplasm.

Findings:

  • Histopathological evaluation confirmed the diagnosis of T-cell PCNSL.
  • The patient received induction chemotherapy with methotrexate and temozolomide.
  • Consolidative radiotherapy was administered following chemotherapy.

Implications:

  • Autoimmune conditions are identified as a risk factor for T-cell PCNSL.
  • T-cell PCNSL shares radiographic and histologic features with gliomas and inflammatory processes.
  • Early diagnosis through extensive histopathological evaluation is crucial for appropriate patient management.
