Summary

Snakebites can rarely cause arterial thrombosis, leading to limb ischemia. This case highlights the importance of considering underlying atherosclerotic disease in snakebite patients to prevent severe outcomes.

Background:

  • Snakebites are a significant global health issue, causing millions of cases and thousands of deaths annually.
  • Common snake envenomation effects include local tissue damage, bleeding disorders, and neurological or kidney toxicity.
  • Rarer complications, such as thrombosis, can arise, particularly when underlying conditions are present.

Observation:

  • A 69-year-old male presented with severe right lower extremity pain 10 days post-snakebite.
  • The patient had no prior history of intermittent claudication or atrial fibrillation.
  • Imaging revealed acute arterial thrombosis in the right popliteal artery, potentially linked to snakebite-induced coagulopathy and pre-existing atherosclerotic occlusive disease.

Findings:

  • The patient underwent emergency catheter-directed thrombolysis for acute limb ischemia.
  • Despite treatment, amputation was necessary due to delayed presentation and irreversible tissue damage.
  • This case illustrates a rare but severe complication of snakebite.

Implications:

  • Physicians must consider acute lower extremity ischemia as a rare snakebite complication.
  • Prompt recognition and management are crucial, especially in patients with underlying atherosclerotic disease.
  • Awareness of potential coagulopathies and vascular events post-snakebite is essential for effective patient care.
