    Summary

    Dietary gluten may trigger IgA immune complexes (IgAIC) in primary IgA nephropathy (PIgAGN). A gluten-free diet significantly reduced IgAIC levels in PIgAGN patients, suggesting a dietary link to this kidney disease.

    Background:

    • Primary IgA nephropathy (PIgAGN) pathogenesis may involve oral immunization mechanisms.
    • IgA immune complexes (IgAIC) are implicated in PIgAGN development.
    • Previous research suggests alimentary antigens can induce IgA nephropathy experimentally.

    Purpose of the Study:

    • To investigate the role of dietary factors, specifically gluten, in primary IgA nephropathy (PIgAGN).
    • To monitor IgA immune complex (IgAIC) levels in PIgAGN patients on controlled diets.
    • To assess the impact of gluten withdrawal and reintroduction on IgAIC levels.

    Main Methods:

    • Six PIgAGN patients with persistent urinary activity and high IgAIC levels were studied.
    • Patients underwent dietary interventions: gluten-free, meat-free, and egg-free diets.

    • A gluten-free diet led to significant decreases in IgAIC levels at 10 days, 1 month, and 6 months.

    • After 6 months on a gluten-free diet, all patients normalized IgAIC values and showed decreased IgA2 subclass-IgAIC.

    Conclusions:

    • A strong relationship exists between gluten consumption and elevated IgAIC levels in PIgAGN patients.
    • Dietary factors, particularly gluten, may play a significant role in the pathogenesis and geographical distribution of PIgAGN.
    • These findings support the hypothesis of an antigen challenge to the mucosal immune system in PIgAGN.
