Area of Science:

  • Medical Education
  • Ultrasound-Guided Procedures
  • Vascular Access

Background:

  • The long-axis in-plane approach for ultrasound-guided central venous catheterization is challenging due to the need for continuous needle visibility.
  • Existing training methods may not adequately address the complexities of this technique.

Purpose of the Study:

  • To evaluate the efficacy of a novel competency-based modular teaching system designed to improve skills in the long-axis in-plane approach for ultrasound-guided central venous catheterization.

Main Methods:

  • A simulator device with an ultrasound probe, simulated vessel, needle, and endoscope was used.
  • Participants underwent pre- and post-training assessments of success rate, needle tip visualization, and puncture accuracy.
  • A 2-hour hands-on training session with the developed system was provided.

Main Results:

  • The overall success rate for central venous catheterization significantly increased from 79% to 94% (P=.04).
  • Needle tip visualization was significantly improved (P=.02).
  • Puncture accuracy, specifically needle tip distance from the vessel center, significantly increased (P=.03).

Conclusions:

  • The developed competency-based modular teaching system effectively enhances needle tip visualization and puncture accuracy.
  • This novel system leads to a significantly higher success rate for ultrasound-guided central venous catheterization using the long-axis in-plane approach.
  • The step-by-step modular design facilitates skill acquisition for this challenging procedure.
