Summary

For midforearm amputations, a second toe transfer can restore meaningful hand function. This innovative approach offers a viable solution for patients previously limited to prosthetics or highly selective transplants.

Background:

Unilateral midforearm amputation presents significant functional deficits.
Current treatments include prosthetic hand restoration or vascularized composite allotransplant (VCA) in select cases.
Distal amputations may benefit from vascularized toe transfers.

Observation:

A case of midforearm amputation is presented.
Vascularized second toe transfer was performed proximal to the pronator teres muscle insertion.
The goal was to achieve functional restoration.

Findings:

A single toe transfer successfully enabled meaningful prehension.
This procedure demonstrated efficacy even at a proximal amputation level.
