Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

647
Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Varicose Veins II: Diagnostic Studies and Interprofessional Care

49
Varicose veins, or varicosities, develop when the valves in the veins, which control blood flow, weaken or damage. It causes blood to pool and the veins to enlarge. Understanding the clinical manifestations, diagnostic approaches, and management options for varicose veins is crucial for effective treatment and relief.Clinical manifestationsClinical manifestations of varicose veins include a heavy, achy feeling or pain after prolonged standing or sitting. This discomfort can often be relieved by...

Summary

Treating valvular endocarditis with cardiac implantable electrophysiological devices (CIED) is complex, especially with reduced ejection fraction. This study details a laser lead extraction technique during venoarterial extracorporeal membrane oxygenation (VA-ECMO) for a challenging case.

Area of Science:

  • Cardiology
  • Cardiac Surgery
  • Electrophysiology

Background:

  • Valvular endocarditis in patients with cardiac implantable electrophysiological devices (CIED) presents complex treatment challenges.
  • Severely reduced left ventricular ejection fraction (LVEF) complicates surgical management and weaning from cardiopulmonary bypass.
  • Postcardiotomy syndrome may necessitate venoarterial extracorporeal membrane oxygenation (VA-ECMO) support.

Purpose of the Study:

  • To describe a novel technique for laser lead extraction in patients with CIED and valvular endocarditis requiring VA-ECMO.
  • To present a case study illustrating the feasibility and application of this technique.

Main Methods:

  • The study describes a technique for laser lead extraction performed during VA-ECMO support.
  • The procedure was applied to a 64-year-old patient with triple valve endocarditis and lead vegetations.

Main Results:

  • Laser lead extraction was successfully performed during VA-ECMO support.
  • The technique enabled the management of lead vegetations in a complex clinical scenario.

Conclusions:

  • Laser lead extraction is a viable option for patients with CIED, valvular endocarditis, and lead vegetations requiring VA-ECMO.
  • This approach can be crucial for managing challenging cases where manual lead extraction is not feasible.
