Summary

The Cognitive Assessment scale for Stroke Patients (CASP) demonstrates good psychometric properties for screening cognitive impairment in stroke survivors. This validated tool aids in assessing cognitive function, even with aphasia or neglect.

Area of Science:

  • Neuroscience
  • Clinical Psychology
  • Rehabilitation Medicine

Background:

  • Standard cognitive screening tools like the Mini Mental State Examination and Montreal Cognitive Assessment can be limited by aphasia or hemispatial neglect in stroke patients.
  • The Cognitive Assessment scale for Stroke Patients (CASP) was developed to address these limitations and has shown superior feasibility.

Purpose of the Study:

  • To verify the psychometric properties of the original French version of the Cognitive Assessment scale for Stroke Patients (CASP).

Main Methods:

  • Principal factorial analysis for structural validity.
  • Convergent and divergent validity assessed by comparing CASP scores with comprehensive neuropsychological assessments and between stroke patients and controls.
  • Internal consistency, reproducibility, and sensitivity to change were evaluated in 201 stroke patients and 50 controls over four visits.

Main Results:

  • CASP demonstrated a one-dimensional structure with confirmed convergent and divergent validity.
  • Good internal consistency and excellent reliability were observed.
  • Responsiveness was moderate, allowing for the estimation of the Minimal Clinically Important Difference (MCID) and predictive thresholds.

Conclusions:

  • The French version of CASP possesses robust psychometric properties for cognitive impairment screening in the subacute post-stroke phase.
  • CASP is suitable for patients with severe motor aphasia or left hemispatial neglect.
  • The scale is not recommended for individuals with severe oral comprehension or visual impairments.
