Summary

Acne keloidalis nuchae (AKN) is a scarring folliculitis. A staged surgical approach with negative-pressure wound therapy and skin grafting offers quicker healing and prevents recurrence in advanced cases.

Background:

  • Acne keloidalis nuchae (AKN) is a chronic scarring folliculitis of the occipital scalp.
  • Characterized by papules, pustules, and eventual tumor-like masses.
  • Medical management is prolonged with frequent recurrences; surgical excision is standard for severe cases.

Observation:

  • A case of advanced Acne keloidalis nuchae was treated with a novel staged surgical approach.
  • The technique involved deep excision, negative-pressure wound therapy (NPWT), split-thickness skin grafting, and further NPWT.
  • This approach was applied to an extensive lesion, one of the largest reported.

Findings:

  • The staged approach resulted in accelerated wound healing compared to secondary intention.
