    Summary

This study evaluated circular stapler use for colorectal anastomosis in 38 patients. Results show 10% anastomotic leakage and 8% stenosis, highlighting the stapler

    Area of Science:

    • Colorectal Surgery
    • Surgical Oncology
    • Gastroenterology

    Background:

    • The circular stapler is a key device for colorectal anastomosis.
    • Assessing its outcomes in specific patient populations and surgical scenarios is crucial for optimizing patient care.

    Purpose of the Study:

    • To evaluate the safety and efficacy of circular stapler use for colorectal anastomosis in Singaporean Chinese patients.
    • To identify risk factors for anastomotic leakage, stenosis, and local recurrence after stapled colorectal resection.

    Main Methods:

    • Retrospective analysis of 38 patients undergoing colorectal anastomosis with a circular stapler.
    • Inclusion criteria: rectal carcinoma (37 cases) and sigmoid diverticulitis (1 case).
    • Data collection included stapler type, cartridge size, anastomotic leakage, stenosis, local recurrence, and patient demographics.

    Related Experiment Videos

    Main Results:

    • The most common cartridge sizes were EEA 28 mm and ILS 29 mm.
    • Clinical anastomotic leakage occurred in 10% of patients with complete resection rings (3/32), exclusively in those with tumors 6-9 cm from the anal verge.
    • Significant stenosis (8%) was observed in three patients, with rectal membrane formation in two. Local recurrence (7/38) within 0.5-3 years was associated with Dukes' C disease and distal bowel clearance <1.5 cm.

    Conclusions:

    • Circular stapler use is effective for colorectal anastomosis, facilitating sphincter conservation in mid-rectal cancer.
    • Optimal outcomes require careful consideration of distal bowel clearance (≥2 cm) and complete pararectal tissue excision.
    • Further research may explore strategies to minimize leakage and stenosis in specific patient subgroups.
