Area of Science:

  • Neurology
  • Gastroenterology

Background:

  • Ulcerative colitis (UC) is an idiopathic autoimmune inflammatory disease.
  • Cerebral sinus vein thrombosis (CVT) is a rare but serious complication of UC.
  • Current CVT management strategies are not well-defined.

Observation:

  • A 23-year-old male with a history of uncontrolled UC presented with headache, light/sound sensitivity, memory loss, and personality changes.
  • Initial head CT revealed left frontoparietal lobe hypodensities.
  • Diagnosis of CVT was confirmed via magnetic resonance angiography and venography.

Findings:

  • The patient received immediate treatment with low-molecular-weight heparin.
  • UC management included methylprednisolone, PPIs, mesalamine, ciprofloxacin, and metronidazole.
  • The patient showed gradual improvement and was discharged on prednisone, azathioprine, levetiracetam, and warfarin.

Implications:

  • Sudden neurological changes in UC patients may indicate CVT.
  • Prompt diagnosis and intervention for CVT can prevent severe neurological sequelae.
  • This case highlights the importance of considering CVT in UC patients presenting with neurological symptoms.
