Orthodontic multibracket appliance therapy significantly increases the risk of developing white spot lesions (WSL) on teeth. This study quantifies the prevalence and extent of WSL before and after treatment.

Background:

  • White spot lesions (WSL) are a common concern during orthodontic treatment.
  • Assessing WSL prevalence and incidence requires accurate diagnostic tools.

Purpose of the Study:

  • To evaluate the prevalence and incidence of WSL before and after multibracket appliance (MB) therapy.
  • To quantify the extent of WSL using a newly developed digital tool and the Enamel Decalcification Index (EDI).

Main Methods:

  • Retrospective analysis of digital photographs from 121 adolescent patients undergoing MB therapy.
  • Evaluation of labial tooth surfaces using the EDI (levels 0-3) and a digital graticule for WSL extent (%).
  • Statistical analysis using crosstabulations and logistic regression.

Main Results:

  • WSL prevalence increased from 69.4% to 97.5% of patients post-MB therapy.
  • Tooth surface involvement with WSL increased from 18.4% to 51.8% after treatment.
  • The extent of WSL (≥20-100%) on tooth surfaces rose from 18.2% to 52.3% post-therapy.

Conclusions:

  • Labial multibracket appliance therapy significantly increases the risk of developing white spot lesions.
  • The developed evaluation index provides a concise quantification of labial WSL extent.
  • Premolars showed the highest incidence of WSL, while lower incisors had the lowest.
