Background:

Primary testicular lymphoma is a rare extranodal lymphoma.
It is an aggressive malignancy with potential for poor prognosis.

Observation:

A 48-year-old male presented with a 10-month history of painless bilateral testicular lumps.
Elevated lactate dehydrogenase (LDH) and ultrasound findings suggested malignancy.
Histopathology confirmed diffuse large B-cell lymphoma.

Findings:

Orchiectomy alone is insufficient for definitive treatment.
Chemotherapy is indicated to improve survival rates.

Implications:

Timely and comprehensive diagnostic workup is essential.
Early treatment improves patient prognosis and outcomes for testicular lymphoma.
