    Background:

    • Radiographic assessment of maxillary molars sometimes reveals a triangular shadow near the furcation.
    • The diagnostic reliability of this "furcation arrow" for proximal furcation involvement requires investigation.

    Purpose of the Study:

    • To evaluate the accuracy of the "furcation arrow" radiographic image in diagnosing proximal furcation involvement in maxillary molars.

    Main Methods:

    • Visual assessment of 66 Degree 1, 53 Degree 2, and 43 Degree 3 furcation involvements in dry skulls using Nyman and Lindhe classification.
    • Radiographic imaging of involved maxillary molars using standard periapical techniques.
    • Independent evaluation of radiographs by six dentists to identify the presence of the "furcation arrow".

    Main Results:

    • A significant association was found between the "furcation arrow" and Degree 2 or 3 furcation involvement compared to uninvolved furcations.
    • The "furcation arrow" appeared equally on mesial and distal furcations and was independent of buccal furcation involvement.
    • Low incidence of the "furcation arrow" in uninvolved and Degree 1 furcations, with no significant difference.

    Conclusions:

    • The presence of the "furcation arrow" suggests proximal bony furcation involvement.
    • Absence of the "furcation arrow" does not exclude furcation involvement, as it was not detected in all advanced cases.
