Summary

Transcatheter aortic valve implantation (TAVI) is standard for aortic stenosis, but post-TAVI thrombosis is an underestimated complication. Its clinical and subclinical impact and optimal treatment, especially anticoagulation for subclinical cases, require further investigation.

Area of Science:

  • Cardiology
  • Interventional Cardiology
  • Biomedical Engineering

Background:

  • Transcatheter aortic valve implantation (TAVI) is a primary treatment for severe symptomatic aortic stenosis across various surgical risk groups.
  • Post-TAVI thrombosis is a recognized complication, presenting with clinical symptoms or as an asymptomatic subclinical finding.
  • Subclinical TAVI thrombosis, characterized by leaflet thickening or cusp thrombosis on CT, affects 10-15% of patients and can impair valve function.

Purpose of the Study:

  • To review the incidence, clinical manifestations, and contributing factors of post-TAVI thrombosis.
  • To discuss the potential role of anticoagulation in managing TAVI thrombosis, particularly subclinical cases.
  • To highlight the underestimation of subclinical TAVI thrombosis and its potential impact on valve performance.

Main Methods:

  • Literature review of studies on TAVI complications, focusing on thrombosis.
  • Analysis of clinical presentations and diagnostic findings (e.g., cardiac CT) of post-TAVI thrombosis.
  • Discussion of etiological factors including biomechanics, patient predispositions, and antithrombotic therapies.

Main Results:

  • Post-TAVI thrombosis occurs in 10-15% of patients, with both symptomatic and asymptomatic (subclinical) forms.
  • Subclinical thrombosis may present as leaflet thickening or cusp thrombosis, potentially affecting valve opening without gradient elevation.
  • Factors implicated include biomechanical issues, patient factors, and antithrombotic treatment regimens.

Conclusions:

  • Post-TAVI thrombosis is a significant complication that is often underestimated, especially in its subclinical form.
  • While anticoagulation (AVK or DOAC) is logical for overt thrombosis, its efficacy in treating subclinical TAVI thrombosis remains uncertain.
  • Further research is needed to clarify the optimal management strategies for different presentations of TAVI thrombosis.
