Background:

  • Noncommunicable diseases (NCDs) present a growing health challenge in Tanzania.
  • Limited healthcare services and urban-rural disparities exacerbate the NCD burden.
  • An integrated management system was piloted in Iringa District to address these issues.

Purpose of the Study:

  • To evaluate the initial 6-month outcomes of an integrated NCD management system.
  • To assess patient follow-up rates and treatment effectiveness for hypertension and diabetes.

Main Methods:

  • Implementation of an integrated management system connecting peripheral health centers and hospitals.
  • Utilized paper-based treatment cards for patient management.
  • Included 542 patients in the initial roll-out phase.

  • 46.1% of patients attended reassessment visits.

  • Addressing barriers like insurance status and geographical access is crucial for system success.
  • Connecting primary care with referral hospitals can enhance patient follow-up and NCD management.
