A rare case of renal hydatidosis (echinococcosis) in an 18-year-old presented with trauma-like symptoms. Prompt diagnosis via imaging and surgical intervention were crucial for managing this parasitic zoonosis.

Background:

  • Cystic echinococcosis (hydatidosis) is a parasitic zoonosis caused by Echinococcus granulosus larvae.
  • Renal hydatidosis is an exceptionally rare manifestation, accounting for only 1-2% of all cases.

Observation:

  • An 18-year-old patient presented with severe lower back pain and gross hematuria following trauma.
  • Imaging revealed cystic lesions consistent with right renal hydatidosis and hemoperitoneum.
  • Clinical deterioration included decreased hematocrit, necessitating surgical intervention.

Findings:

  • Surgical exploration revealed extensive cysts within and surrounding the right kidney, with no viable parenchyma.
  • A total nephrectomy was performed, and pathological analysis confirmed hydatidosis.
  • The patient received albendazole treatment post-surgery.

Implications:

  • Surgery remains the primary treatment for renal hydatidosis.
  • Ultrasonography is crucial for early zoonosis detection, particularly in pediatric cases.
  • Early diagnosis facilitates more conservative therapeutic approaches.
