Summary

Active surgical intervention improves long-term outcomes for retroperitoneal liposarcoma patients. While high-grade tumors and multiple relapses pose challenges, surgery remains the primary radical treatment, with extensive resection history correlating with better survival.

Background:

  • Retroperitoneal liposarcoma is a rare and challenging malignancy.

Purpose of the Study:

  • To evaluate the impact of an active surgical approach on long-term postoperative results in patients diagnosed with retroperitoneal liposarcoma.

Main Methods:

  • A retrospective analysis was conducted on 190 patients with retroperitoneal liposarcoma.
  • Key factors analyzed included malignancy grade, adjuvant chemotherapy, tumor node multiplicity, and prior resection history.
  • Survival rates were assessed in relation to these variables.

Main Results:

  • High-grade (G2-3) retroperitoneal liposarcoma significantly correlated with worse overall and relapse-free survival compared to low-grade (G1) tumors.
  • Multifocal primary tumors and multiple metastatic nodes at first relapse did not significantly impact prognosis post-resection.
  • Adjuvant chemotherapy did not significantly enhance relapse-free survival.
  • Patients with a history of four or more previous total resections demonstrated significantly higher overall survival.

Conclusions:

  • Surgery is the sole potentially curative treatment for retroperitoneal liposarcoma.
  • An aggressive surgical strategy is associated with improved long-term outcomes for patients with this condition.
