Mitochondrial markers did not differ between major depressive disorder (MDD) patients and controls. However, higher baseline mitochondrial content and complex I activity distinguished SSRI responders from nonresponders, suggesting potential biomarkers for treatment response.

- Assessed SSRI treatment response (n=33) using >50% decrease on the Hamilton Depression Rating Scale (HRDS-25) after 8 weeks.

- No significant differences in MHI or other mitochondrial markers were found between MDD subjects and HCs.
- SSRI responders showed significantly higher baseline CS, mtDNAcn, and complex I activity compared to nonresponders.
- Complex I activity changes correlated with treatment response, decreasing in responders and increasing in nonresponders.
Conclusions:
- Mitochondrial markers did not differentiate MDD patients from controls but could distinguish SSRI responders from nonresponders.
- These findings suggest mitochondrial markers may serve as biomarkers for predicting SSRI treatment efficacy and identifying novel therapeutic targets for MDD.
