Mitochondrial markers did not differ between major depressive disorder (MDD) patients and controls. However, higher baseline mitochondrial content and complex I activity distinguished SSRI responders from nonresponders, suggesting potential biomarkers for treatment response.

Background:

  • Mitochondrial dysfunction is linked to major depressive disorder (MDD).
  • The Mitochondrial Health Index (MHI) previously correlated with stress and emotional states.

Purpose of the Study:

  • To investigate mitochondrial respiratory chain (RC) activities, mitochondrial DNA copy number (mtDNAcn), and MHI in unmedicated MDD subjects versus healthy controls (HC).
  • To explore if these mitochondrial markers predict response to SSRI treatment in MDD patients.

Main Methods:

  • Assayed mitochondrial RC complexes I, II, IV, citrate synthase (CS), mtDNAcn, and MHI in peripheral blood mononuclear cells from 47 MDD subjects and 11 HC.
  • Assessed SSRI treatment response (n=33) using >50% decrease on the Hamilton Depression Rating Scale (HRDS-25) after 8 weeks.

Main Results:

  • No significant differences in MHI or other mitochondrial markers were found between MDD subjects and HCs.
  • SSRI responders showed significantly higher baseline CS, mtDNAcn, and complex I activity compared to nonresponders.
  • Complex I activity changes correlated with treatment response, decreasing in responders and increasing in nonresponders.

Conclusions:

  • Mitochondrial markers did not differentiate MDD patients from controls but could distinguish SSRI responders from nonresponders.
  • These findings suggest mitochondrial markers may serve as biomarkers for predicting SSRI treatment efficacy and identifying novel therapeutic targets for MDD.
