Background:

Inverted urothelial papilloma (IUP) is an uncommon urothelial neoplasm.
While typically found in the bladder, IUP in the upper urinary tract is rare, with limited case documentation.

Observation:

A rare case of inverted urothelial papilloma (IUP) was observed in the distal left ureter.
The patient presented with symptoms necessitating surgical intervention.

Findings:

The patient underwent a distal ureterectomy.
A successful ureteral reimplant with a psoas hitch procedure was performed.

Implications:

This case contributes to the limited literature on upper tract IUP.
