Summary

Percutaneous lithotripsy achieved stone-free kidneys in 64% of 132 patients. Larger stones, especially staghorn calculi, were associated with higher failure rates in this early experience.

Area of Science:

  • Urology
  • Nephrology
  • Minimally Invasive Surgery

Background:

  • Percutaneous lithotripsy (PL) emerged as a key treatment for kidney stones.
  • Early adoption of PL required evaluation of its efficacy and safety.

Observation:

  • A review of 132 percutaneous lithotripsy cases from 1984-1985 was conducted.
  • Data included stone clearance rates, residual stone burdens, and complication profiles.

Findings:

  • Complete stone clearance was achieved in 64% of cases (86/132).
  • Significant residual stones or fragments occurred in 12 cases, with 12 others requiring open surgery.
  • Stones larger than 20 mm and staghorn calculi were associated with increased failure rates.
  • Minor complications occurred in 29 patients, with 7 requiring conversion to open surgery.

Related Experiment Videos

  • Five severe complications were noted, including one fatality from late bleeding complications.
  • Implications:

    • Percutaneous lithotripsy demonstrated potential for kidney stone management but highlighted challenges with complex stones.
    • Early PL series indicated a learning curve and the need for careful patient selection.
    • Complication rates, though manageable, underscored the risks associated with the procedure.
