Insights

Juvenile idiopathic arthritis patients may have undiagnosed celiac disease. Screening for celiac disease (CD) in children with JIA is crucial, even without symptoms, as CD can be confirmed pathologically.

Area of Science:

  • Pediatric Rheumatology
  • Gastroenterology
  • Immunology

Background:

  • A potential link exists between celiac disease (CD) and juvenile idiopathic arthritis (JIA).
  • Evaluating the serological incidence of CD in JIA patients is important for early detection and management.

Purpose of the Study:

  • To determine the serological incidence of celiac disease (CD) in pediatric patients diagnosed with juvenile idiopathic arthritis (JIA).
  • To assess the clinical manifestations of CD in this patient cohort.

Main Methods:

  • Seventy-eight children under 16 with JIA, inadequately responding to treatment, were enrolled from Tehran University of Medical Sciences (2017-2019).
  • CD-related serological screening tests, including Anti-TTG-Ab, were measured.
  • Clinical manifestations of CD were evaluated.

Main Results:

  • Three patients (3.8%) with oligoarticular JIA showed elevated Anti-TTG-Ab levels, with no reported CD symptoms.
  • No significant differences were observed in growth disorders, sex distribution, or JIA subtypes between sero-positive and sero-negative groups.
  • Celiac disease was pathologically confirmed in one patient, necessitating a gluten-free diet.

Conclusions:

  • The study highlights a 3.8% serological prevalence of celiac disease in children with JIA.
  • Absence of gastrointestinal symptoms does not exclude concomitant celiac disease in patients with juvenile idiopathic arthritis.
  • Routine serological screening for CD is recommended in JIA patients, irrespective of clinical presentation.

Enzyme-Linked Immunosorbent Assay

In 1971, Peter Perlman and Eva Engvall developed an Enzyme-linked immunosorbent assay (ELISA or EIA). ELISA differs from western blot in that the assays are conducted in microtiter plates or in vivo rather than on an absorbent membrane.
There are many different types of ELISAs, but they all involve an antibody molecule whose constant region binds an enzyme, leaving the variable region free to bind its specific antigen.  Enzyme-substrate reaction allows the antigen to be visualized or...

Inflammatory Bowel Disease III: Diagnostic Studies and Management I-Nutritional Therapy

Various diagnostic tests are employed in the diagnostic process for Inflammatory Bowel Disease (IBD), particularly to differentiate between Crohn's disease and ulcerative colitis.

A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the...

Myasthenia Gravis: Diagnostic Tests

Myasthenia gravis is an autoimmune condition affecting neuromuscular transmission, causing generalized weakness in skeletal muscles. Initial diagnoses rely on patients' signs, symptoms, and medical history. The challenge lies in distinguishing myasthenia from other muscular dystrophies. An important diagnostic feature is the significant improvement of symptoms after administering anticholinesterase inhibitors.
The edrophonium test is a diagnostic tool for myasthenia gravis. It involves...

Autoimmune Disorders

Autoimmune diseases are a group of disorders in which the body's immune system mistakenly attacks its own cells, tissues, and organs. This results from an overactive immune response against substances and tissues normally present in the body. Let's delve into the concept and mechanism of autoimmune diseases from an immune system point of view, explore different causes and examples of such diseases, and discuss potential solutions.

Drugs for Treatment of Crohn's Disease in IBD Using Immunomodulatory Agents

Crohn's disease is an inflammatory bowel disorder marked by chronic inflammation of the GI tract. Various treatment strategies for Crohn's disease are employed, such as immunomodulatory agents, glucocorticoids, and biologics or anti-TNF therapy. Azathioprine (Imuran), a commonly used immunomodulatory drug for Crohn's disease, is converted in the body to mercaptopurine, which inhibits purine biosynthesis and cell proliferation. Both are utilized in severe cases of Inflammatory Bowel...
