Summary

This study found that using a semi-closed circle anesthesia system with the vaporizer out-of-circle is safe for maxillofacial surgery. This method allows predictable halogenated agent concentrations and offers economic and environmental benefits.

Area of Science:

  • Anesthesiology
  • Respiratory Therapy

Context:

  • Maxillofacial surgery requires controlled ventilation and precise anesthetic delivery.
  • Semi-closed circle anesthesia systems offer potential benefits but require careful monitoring.

Purpose:

  • To evaluate the safety and efficacy of a semi-closed circle anesthesia system with the vaporizer placed out-of-circle during maxillofacial procedures.
  • To establish the correlation between delivered and inspired halogenated anesthetic concentrations.

Summary:

  • Thirty-six patients undergoing maxillofacial surgery were ventilated using a semi-closed circle system with fresh gas flow at 1,200 ml/min.
  • The vaporizer was positioned out-of-circle, with halothane for maintenance and isoflurane for hypotension. Inspired concentrations were monitored.
  • A linear correlation was found between delivered and inspired halogenated agent concentrations, with inspired levels consistently lower than delivered. Oxygen monitoring and carbon dioxide absorption were also assessed.

Related Experiment Videos

Impact:

  • The system demonstrated safety, allowing predictable inhaled anesthetic concentrations.
  • Significant economies in anesthetic agents (nitrous oxide and halogenated agents) were achieved.
  • Prevention of operating room contamination and improved gas humidification were noted advantages.
