Abstract

Insights

Targeted therapies show clinical benefit for lung squamous cell carcinomas (LUSCs) with EGFR or ALK alterations, particularly in never-smoker females. However, progression-free survival is shorter than in lung adenocarcinomas (LUADs).

Area of Science:

  • Oncology
  • Molecular Biology
  • Genetics

Background:

  • Targeted therapies are effective for oncogene-driven lung adenocarcinomas (LUADs).
  • The efficacy of these therapies in EGFR-mutant or ALK-rearranged lung squamous cell carcinomas (LUSCs) is less understood due to rarity.

Purpose of the Study:

  • To investigate the clinical benefit and outcomes of tyrosine kinase inhibitors (TKIs) in patients with metastatic LUSC harboring EGFR or ALK alterations.
  • To compare the comutation profiles of EGFR-mutant LUAD and LUSC.

Main Methods:

  • Retrospective review of metastatic LUSC cases with EGFR or ALK alterations at MD Anderson Cancer Center.
  • Analysis of patient demographics, treatment response, and progression-free survival (PFS) following TKI therapy.
  • Comparison of comutation patterns between EGFR-mutant LUAD (TCGA) and LUSC.

Main Results:

  • Eight EGFR-mutant and six ALK-rearranged LUSC patients received TKIs; 79% were female and 86% never-smokers.
  • Partial response or stable disease was achieved in 7/8 EGFR-mutant and 4/6 ALK-rearranged LUSC cases.
  • Median PFS was 4.9 months for EGFR-mutant LUSC and 2.9 months for ALK-rearranged LUSC. LUSC showed distinct comutation patterns, with higher PIK3CA and KRAS/BRAF alterations compared to LUAD.

Conclusions:

  • EGFR or ALK alterations are present in LUSC, especially in never-smoker females.
  • TKIs provide clinical benefit in disease control for these LUSC patients.
  • The duration of benefit (PFS) is significantly shorter in LUSC compared to LUAD.
