Background:

  • Entecavir (ETV) is a nucleoside analogue widely used for treating chronic hepatitis B virus (HBV) infection.
  • Entecavir-associated thrombocytopenia is infrequently reported and typically considered an immediate adverse reaction.
  • Management often involves discontinuing ETV and avoiding other nucleoside analogues.

Observation:

  • A 66-year-old female HBV patient developed severe thrombocytopenia (platelet count dropped to 3*10^9/L) after 88 days of ETV treatment.
  • The patient presented with gum bleeding and skin ecchymosis, indicating a delayed-onset adverse reaction.
  • Other causes of thrombocytopenia were excluded through comprehensive blood and bone marrow examinations.

Findings:

  • This case represents the third report of entecavir-associated thrombocytopenia with a delayed response.
  • Switching treatment from entecavir to tenofovir (TDF) was effective in managing the thrombocytopenia.
  • Concurrent treatments included platelet transfusions, thrombopoietin, and prednisone, leading to platelet count improvement within 10 days.

Implications:

  • Regular platelet count monitoring is crucial for patients receiving entecavir therapy.
  • Tenofovir disoproxil fumarate (TDF) may be a feasible alternative antiviral treatment option for patients who develop entecavir-associated thrombocytopenia.
  • This case expands the understanding of the potential delayed-onset nature of ETV-induced thrombocytopenia.
