PCSK9 inhibitors dramatically improved non-alcoholic steatohepatitis (NASH) and liver histology in a patient with familial hypercholesterolemia. This suggests PCSK9 inhibitors may be a promising therapy for NASH.

  • Elevated LDL cholesterol levels are independently linked to an increased risk of NAFLD.

Observation:

  • A case report details a 39-year-old woman with acute NASH and familial hypercholesterolemia, resistant to lifestyle changes and statins.
  • Rhabdomyolysis precluded further statin use, necessitating alternative treatments.
  • Liver biopsy confirmed acute NASH, characterized by microvesicular and macrovesicular steatosis with ballooning degeneration.

Findings:

  • The patient received PCSK9 inhibitors as salvage therapy, administered monthly.
  • Within three months, significant improvements were observed: an 80% reduction in ALT and AST levels and a 48% decrease in LDL levels.
  • An eight-month follow-up liver biopsy revealed normalization of liver histology.

  • PCSK9 inhibitors demonstrated a remarkable therapeutic response in a patient with severe NASH who failed conventional treatments.
  • These findings highlight the potential of PCSK9 inhibitors as a novel therapeutic strategy for NASH.
