Summary

Remote pulmonary rehabilitation (PR) offers a feasible and effective alternative to traditional in-person programs for chronic lung disease patients. These alternative PR models provide similar benefits, overcoming barriers to access and improving patient outcomes.

Background:

  • Traditional pulmonary rehabilitation (PR) is underutilized by chronic lung disease patients due to access barriers, exacerbated by the COVID-19 pandemic.
  • Alternative PR delivery models, including home-based, tele-rehabilitation, and web-based programs, are emerging to address these limitations.

Purpose of the Study:

  • To determine if alternative, remotely based pulmonary rehabilitation (PR) delivery models are as effective as traditional clinic-based PR.
  • To evaluate the feasibility and efficacy of tele-rehabilitation for patients with chronic lung disease.

Main Methods:

  • A literature review of randomized controlled trials (RCTs), cohort studies, and case series was conducted.
  • Searches were performed in PubMed, CINAHL, and Medline up to May 1, 2021, identifying 26 applicable studies.
  • Studies evaluated tele-rehabilitation against conventional PR, baseline status, or no rehabilitation.

Main Results:

  • Tele-rehabilitation was found to be a feasible and efficacious option for select patients with lung disease.
  • Outcomes from remotely based PR demonstrated benefits comparable to traditional PR programs.
  • The data does not indicate that remotely based PR diminishes the effectiveness of conventional PR.

Conclusions:

  • Remotely based PR is a viable and effective option, particularly for patients facing barriers to conventional PR.
  • Further high-quality RCTs are necessary to fully ascertain the efficacy, safety, cost-effectiveness, and optimal patient selection for remotely based PR.
