Background:

  • Subacute thyroiditis (SAT) is a common, self-limiting thyroid disorder characterized by neck pain, often linked to viral infections.
  • While typically associated with viral infections, SAT following vaccination is exceptionally rare.

Observation:

  • A patient developed fever, neck pain, and fatigue approximately 8 hours after receiving the first dose of the 9-valent human papillomavirus vaccine (Gardasil 9).
  • Physical examination revealed a tender, enlarged thyroid gland. Laboratory tests indicated elevated inflammatory markers and thyroid antibodies.
  • Thyroid ultrasonography showed diffuse bilateral thyroid enlargement with an uneven echo pattern, leading to a diagnosis of SAT.

Findings:

  • The patient's symptoms, including fever and neck pain, significantly improved within 5 days of ibuprofen treatment.
  • Following the complete vaccination series, thyroid function normalized, and SAT did not recur during follow-up.

Implications:

  • This case highlights the potential, albeit rare, association between the 9-valent human papillomavirus vaccine and subacute thyroiditis.
  • It underscores the importance of considering vaccination as a potential trigger for SAT in clinical practice.
  • The successful management and lack of recurrence suggest that SAT post-vaccination may be transient and manageable.
