  • Posterior urethral valves (PUV) are congenital urethral obstructions in males.
  • Presentation varies from antenatal hydronephrosis to postnatal voiding dysfunction and renal anomalies.

Observation:

  • An 11-day-old male presented with poor feeding, vomiting, abdominal distension, and edema.
  • The infant exhibited hypoglycemia, hyponatremia, hyperkalemia, and acute renal failure.
  • Point-of-care ultrasound revealed severe hydronephrosis and a distended bladder compressing the inferior vena cava.

Findings:

  • Ultrasound identified severe bilateral hydronephrosis and bladder outlet obstruction.
  • The obstruction led to significant electrolyte imbalances and acute kidney injury.
  • Bladder decompression via catheterization was performed urgently.

Implications:

  • Early recognition of PUV through point-of-care ultrasound is vital.
  • Prompt bladder decompression can prevent severe complications.
  • Multidisciplinary collaboration (urology, nephrology, neonatology) is essential for optimal outcomes.
