Background:

Cervical thymomas are exceptionally rare tumors.
Distinguishing them from other neck masses, like thyroid nodules or lymphomas, presents diagnostic challenges.

Observation:

A 45-year-old woman presented with a five-year history of neck swelling and dysphagia.
Initial fine-needle aspiration suggested Hashimoto's thyroiditis; intraoperative frozen section suggested lymphoma.

Findings:

Surgical excision revealed a mass at the cricoid cartilage displacing the thyroid.
Histopathology and immunohistochemistry confirmed a thymoma with T-cell phenotype and both lymphoid and epithelial cells.

Implications:

This case underscores the diagnostic difficulties associated with cervical thymomas.
Cervical thymoma may be an underdiagnosed entity due to its rarity and mimicry of common neck pathologies.
