  • Magnetic resonance imaging revealed a calcified mass compressing the spinal canal at L4-L5.

Findings:

  • Despite initial treatments, the patient developed polyarticular arthritis and persistent systemic inflammation.
  • Diagnosis shifted to acute polyarticular gout flare with a severe systemic inflammatory reaction.
  • Treatment with colchicine and anti-inflammatory drugs led to symptom resolution and normalization of inflammatory markers.

Implications:

  • This case underscores the diagnostic difficulty of acute polyarticular gout, especially with atypical joint involvement and severe systemic reactions.
  • Recognizing gout as a cause of severe systemic inflammation is crucial, even with unusual presentations.
  • The term "gout storm" is proposed for these severe, systemic gout flares, emphasizing the need for timely diagnosis and treatment.
