    Electroconvulsive therapy (ECT) did not show lasting brain changes in patients. Brain imaging revealed enlarged ventricles in patients, but this was linked to affective disorders, not ECT itself.

    Background:

    • Electroconvulsive therapy (ECT) is widely used for psychiatric disorders.
    • Concerns persist regarding potential long-term neurological effects of ECT.
    • Previous studies have yielded conflicting results on ECT's impact on brain structure.

    Purpose of the Study:

    • To investigate potential morphological brain changes in patients after electroconvulsive therapy (ECT).
    • To compare brain imaging findings between ECT patients, non-ECT psychiatric patients, and healthy controls.

    Main Methods:

    • 22 patients with ECT history underwent clinical evaluation and brain CT scans.
    • Comparison with age- and sex-matched controls (non-ECT patients and healthy volunteers).
    • Analysis of ventricle/brain ratios and cortical atrophy scores on CT scans.

    Main Results:

    • Patients exhibited larger ventricle/brain ratios and cortical atrophy scores than healthy controls.
    • No significant association was found between these radiological measures and ECT history.
    • Age correlated significantly with ventricle/brain ratios and atrophy scores across all subjects.
    • Correlations with illness duration and psychotropic drug exposure were inconclusive.

    Conclusions:

    • Findings suggest enlarged brain ventricles may be associated with affective disorders, not ECT.
    • The clinical significance of observed ventricular enlargement in psychiatric patients requires further investigation.
    • ECT does not appear to cause lasting morphological brain changes detectable by CT scan in this cohort.
