Summary

Pre-LITT biopsies can cause air/blood artifacts, leading to signal dropout that interferes with laser interstitial thermal therapy (LITT) ablation monitoring. Further research is needed to mitigate these effects.

Background:

  • Laser interstitial thermal therapy (LITT) safety and effectiveness depend on real-time temperature mapping via magnetic resonance thermometry (MRT).
  • Gradient recalled echo (GRE) sequences used in MRT are sensitive to susceptibility effects from air and blood.
  • Pre-LITT biopsies are anecdotally linked to ablation artifacts.

Purpose of the Study:

  • To review the impact of pre-LITT biopsies on MRI artifact during ablation.
  • To describe signal dropout that can interfere with LITT monitoring.

Main Methods:

  • Retrospective review of 17 LITT cases for brain tumors (2017-2020).
  • Analysis of cases for biopsy presence, artifact, operative technique, and post-operative air/blood detection.

Main Results:

  • Six biopsy cases all showed artifact during ablation and had air/blood post-operatively.
  • Artifactual signal dropout interfered with thermal damage thresholds in two biopsy cases.
  • Eleven non-biopsy cases had no artifact or significant post-ablation air/blood.

Conclusions:

  • Pre-LITT biopsies can introduce air/blood, causing significant artifactual signal dropout.
  • This dropout can interfere with effective LITT ablation.
  • Strategies to modify these effects require further investigation.
