    Background:

    • Iron is essential for brain function but its precise distribution is complex.
    • Understanding normal brain iron distribution is crucial for diagnosing neurological disorders.

    Observation:

    • 150 individuals showed decreased T2-weighted MRI signal in the globus pallidus, substantia nigra, red nucleus, and dentate nucleus.
    • This signal decrease correlated with reduced T2 relaxation times and iron accumulation sites identified by Perls staining.

    Findings:

    • T2-weighted MRI provides an accurate in vivo map of normal brain iron distribution.
    • Iron levels are highest in the basal ganglia, intermediate in the striatum, and lower in cerebral gray and white matter.
    • White matter iron distribution varies, being absent in posterior internal capsule/optic radiations, higher frontally, and prominent in subcortical U-fibers.

    Implications:

    • Brain iron increases with age, starting from undetectable levels at birth.
    • Mapping brain iron aids in understanding normal anatomy and neurodegenerative, demyelinating, and cerebrovascular disorders.
