Background:

  • It is an infrequent complication following laparoscopic nephrectomy, leading to malnutrition and immunodeficiency due to nutrient and antibody loss.

Observation:

  • A 55-year-old female presented with abdominal distension and constipation two weeks post-left laparoscopic donor nephrectomy.
  • Abdominal ultrasound confirmed free fluid, and paracentesis revealed chylous ascites.

Findings:

  • Initial conservative treatment, including a high-protein, low-fat diet, proved ineffective.
  • Subsequent lymphangiography and embolization of the lymphatic defect successfully resolved the chylous ascites.
