Area of Science:

  • Neuroscience
  • Cognitive Psychology
  • Gerontology

Background:

  • The One Card Learning Test (OCL80) is a digital cognitive assessment tool used in clinical trials and healthcare.
  • While sensitive to early Alzheimer's disease (AD) memory changes, its efficacy in symptomatic AD is debated compared to other memory tests.
  • This study addresses the need for improved sensitivity in cognitive assessments for AD.

Purpose of the Study:

  • To develop and validate a modified version of the OCL80 (OCL48) with enhanced sensitivity to memory impairment in Alzheimer's disease.
  • To assess the reliability and validity of the OCL48 in different populations and testing conditions.

Main Methods:

  • Modified the OCL80 to create the OCL48, reducing test difficulty while preserving core pattern separation operations.
  • Conducted experiments with healthy adults and individuals with AD to evaluate performance, reliability, and sensitivity.
  • Utilized equipercentile equating to calibrate scores between OCL80 and OCL48 using data from cognitively normal older adults.

Main Results:

  • The OCL48 demonstrated improved performance in healthy adults without compromising pattern separation.
  • Repeated OCL48 administration showed no significant ceiling or practice effects.
  • The OCL48 exhibited substantially greater sensitivity to AD-related memory impairment (Glass's Δ = 3.11) compared to the OCL80 (Glass's Δ = 1.94).

Conclusions:

  • The OCL48 is a valid and reliable learning test with superior sensitivity to memory deficits in Alzheimer's disease compared to the OCL80.
  • This modified digital cognitive test holds promise for more accurate early detection and monitoring of AD.
  • The OCL48 offers an improved tool for clinical trials and healthcare decision-making in AD.
