Background:

Venom-induced consumption coagulopathy (VICC) typically involves decreased coagulation factors and fibrinogen (FBG).
This case presents a unique VICC manifestation following a snake bite.

Observation:

A 49-year-old male presented with persistent bleeding and subcutaneous hemorrhage post-snake bite.
Initial diagnosis of VICC was confirmed by laboratory findings and thromboelastography.
The patient exhibited persistent afibrinogenemia despite normal coagulation factor activity.

Findings:

Treatment with cryoprecipitate, fresh frozen plasma, and human FBG resolved symptoms.
Laboratory abnormalities normalized by day 25.
No recurrent hemorrhagic events occurred during 6 months of follow-up.

Implications:

Hemorrhagic snake venom can cause VICC with isolated, persistent FBG deficiency.
This presentation challenges typical VICC diagnostic criteria.
Highlights the importance of considering FBG levels in VICC diagnosis and management.
