Background:

  • Autoimmune hypophysitis is a rare inflammatory condition affecting the pituitary gland, often causing hypopituitarism and pituitary mass.
  • Current management involves hormonal replacement, anti-inflammatory drugs, surgery, or radiotherapy for mass-related symptoms.

Observation:

  • A 25-year-old male presented with visual field defects, headache, and weight loss, indicative of a sellar mass.
  • Histopathology confirmed autoimmune hypophysitis with significant CD20-positive B-cell infiltration.
  • Initial treatment with prednisolone and azathioprine failed to prevent relapse after dose reduction.

Findings:

  • Rituximab treatment led to significant regression of the pituitary mass in a patient with B-cell predominant autoimmune hypophysitis.
  • The patient achieved sustained remission for 4 years after discontinuing rituximab after 25 months of therapy.

Implications:

  • Rituximab may represent an effective therapeutic alternative for autoimmune hypophysitis, particularly in cases with B-cell predominance.
  • This finding could guide future treatment strategies for refractory or relapsing autoimmune hypophysitis.
