    Summary

    Gastrointestinal bleeding risks from piroxicam are similar to other nonsteroidal anti-inflammatory drugs (NSAIDs). A prospective study found no significant difference in bleeding events associated with piroxicam compared to other NSAIDs.

    Background:

    • Spontaneous reports suggested a link between piroxicam and gastrointestinal bleeding in Norway.
    • Retrospective studies indicated aspirin was more frequently associated with these bleeding events.

    Purpose of the Study:

    • To prospectively investigate the association between piroxicam use and gastrointestinal bleeding.
    • To compare the risk of gastrointestinal adverse effects among various nonsteroidal anti-inflammatory drugs (NSAIDs).

    Main Methods:

    • A 14-month prospective case study in Tromso, Norway.
    • Inclusion criteria: patients hospitalized for hematemesis, melena, or perforation.

    Main Results:

    • Of 43 patients with gastrointestinal bleeding, 21 used aspirin or aspirin-like drugs.
    • Among 22 patients using other NSAIDs, adverse effects were evenly distributed.
    • Concomitant use of aspirin or steroids, and history of ulcers/dyspepsia, were common.

    Conclusions:

    • Gastrointestinal side effects are associated with all NSAIDs.
    • Piroxicam's association with gastrointestinal bleeding is not significantly different from other NSAIDs.
