    Summary

    Serum bone gla-protein (SBGP) levels indicate bone formation in multiple myeloma (MM). Lower SBGP correlates with advanced MM, suggesting osteoblastic inhibition, but some patients show coupled bone remodeling. SBGP may be a promising marker for MM evaluation.

    Background:

    • Multiple myeloma (MM) is a plasma cell malignancy characterized by bone destruction.
    • Understanding bone remodeling dynamics in MM is crucial for patient management.
    • Serum bone gla-protein (SBGP) is a key marker of bone formation.

    Purpose of the Study:

    • To investigate the role of SBGP as a marker of bone formation in patients with multiple myeloma.
    • To correlate SBGP levels with clinical features and disease activity in MM.
    • To explore the relationship between SBGP and bone remodeling processes (coupling/uncoupling) in MM.

    Main Methods:

    • Radioimmunoassay was used to measure SBGP levels in 57 MM patients.
    • SBGP levels were compared to normal controls and patients with monoclonal gammopathy of undetermined significance.
    • Correlation analysis was performed between SBGP levels and disease stage, bone lesions, hypercalcemia, and disease progression.

    Main Results:

    • While overall SBGP levels did not differ significantly between MM patients and controls, 18% of MM patients had abnormal values.

    • SBGP levels inversely correlated with disease severity, showing lowest values in patients with extensive lytic bone lesions and hypercalcemia, indicating osteoblastic inhibition in 80% of active MM cases. A subset (20%) exhibited coupled bone remodeling with increased SBGP.
    • Serial studies showed SBGP levels correlated with disease activity and returned to normal during remission induction.

    Conclusions:

    • SBGP is a promising marker for assessing bone formation and disease activity in multiple myeloma.
    • The study identified distinct patterns of bone remodeling (uncoupled vs. coupled) in MM patients based on SBGP levels.
    • Changes in SBGP levels during treatment reflect disease status and response to therapy.
