Background:

Morbid obesity is linked to infertility, with weight loss often improving male fertility.
Bariatric surgery is a critical intervention for severe obesity, offering significant health benefits.

Observation:

Two male patients developed azoospermia (absence of sperm) following bariatric surgery.
One patient had a BMI of 81.2 kg/m² and the other 52 kg/m², with no prior fertility issues.

Findings:

Bariatric surgery can paradoxically lead to decreased sperm parameters, including azoospermia.
This may result from a combination of reduced obesity impact and nutrient deficiencies post-surgery.

Implications:

Male patients undergoing bariatric surgery should be counseled on the potential risk of temporary or permanent infertility.
Sperm cryopreservation should be discussed as a proactive measure for fertility preservation before or after surgery.
