    Area of Science:

    • Pharmacology
    • Toxicology
    • Psychiatry

    Background:

    • Orphenadrine is commonly prescribed for neuroleptic extrapyramidal side effects.
    • A concerning trend of increased mortality due to orphenadrine poisoning has been observed.

    Observation:

    • The study highlights a rise in deaths, predominantly among severely psychotic males.
    • Lethal doses of orphenadrine are alarmingly close to the average weekly therapeutic dose.

    Findings:

    • Orphenadrine poisoning presents with severe, rapid-onset cardiac and neurological symptoms.
    • Even small overdoses (1-2 g) can be life-threatening, necessitating urgent medical attention.

    Implications:

    • Restricting access to orphenadrine for individuals with suicidal ideation is strongly recommended.
    • Immediate intensive care unit (ICU) observation should be initiated for all suspected orphenadrine overdoses.
