Choroidal metastasis (CM) from esophageal cancer is rare, presenting as retinal detachment. Early diagnosis and a multidisciplinary approach are crucial for managing this rare ocular complication.

Background:

Choroidal metastasis (CM) is a rare but serious complication of cancer, most commonly originating from breast carcinoma.
Esophageal cancer, while less common, can also metastasize to the choroid, leading to significant ocular morbidity.

Observation:

A 62-year-old man with resected esophageal cancer and brain metastasis developed choroidal metastasis with exudative retinal detachment in the left eye.
Despite initial improvement with chemotherapy, visual acuity declined after brain surgery, revealing multiple choroidal masses and retinal detachment.

Findings:

The patient presented with recurrent exudative retinal detachment without retinal tears, ultimately diagnosed as choroidal metastasis.
Despite recommendations for enucleation, the patient refused treatment, and systemic metastases led to his death.

Implications:

This case highlights the importance of considering choroidal metastasis in patients with known cancer and new-onset retinal detachment.
Developing standardized treatment plans and a multidisciplinary approach is essential for early diagnosis and timely intervention in rare ocular metastases.
