    Background:

    • Urinary tract infections (UTIs) are common, particularly in women.
    • Identifying causative agents is crucial for effective treatment.
    • The role of fastidious organisms in UTIs requires further investigation.

    Purpose of the Study:

    • To compare the prevalence of conventional and fastidious organisms in acutely dysuric women versus asymptomatic controls.
    • To assess the association between organism isolation and pyuria.

    Main Methods:

    • Urine samples from 185 dysuric women and 89 controls were cultured using conventional and microaerophilic methods.
    • Organisms were identified and quantified.
    • Pyuria was assessed in patients with identified infections.

    Main Results:

    • Coliform bacilli were isolated from 67.5% of symptomatic women.
    • Fastidious organisms were isolated from 13.4% of symptomatic women and 4.5% of controls (P=0.055).
    • Pyuria was present in all conventional infections but only 30.4% of fastidious organism infections.

    Conclusions:

    • Fastidious organisms may play a role in dysuria, even in the absence of significant pyuria.
    • Conventional culture methods may miss certain UTI pathogens.
    • Further research is needed to elucidate the clinical significance of fastidious organisms in UTIs.
