    Background:

    • Rheumatoid arthritis (RA) is a complex autoimmune disease with a known genetic component.
    • Complement system proteins, including Properdin factor B (Bf) and C3, play roles in immune response and inflammation.
    • Genetic polymorphisms in immune-related genes are investigated for associations with RA susceptibility and clinical manifestations.

    Purpose of the Study:

    • To investigate the association of Properdin factor B (Bf) and complement C3 polymorphisms with rheumatoid arthritis (RA) in a North-East England cohort.
    • To explore potential genetic heterogeneity within RA subgroups defined by rheumatoid factor (RF) and antinuclear antibody (ANA) status.

    Main Methods:

    • Genotyping of Properdin factor B (Bf) and complement C3 polymorphisms in 225 unrelated RA patients.
    • Subdivision of patients based on the presence or absence of significant rheumatoid factor and antinuclear antibody titres.
    • Statistical analysis to detect associations between Bf and C3 phenotypes and RA patient subgroups.

    Main Results:

    • No significant association was found between complement C3 polymorphisms and RA.
    • A significant excess of the Bf SS phenotype and a deficiency of the Bf FS phenotype were observed in RA patients who were seropositive and ANA-negative.
    • These findings suggest a specific genetic link between Bf polymorphisms and a particular subgroup of RA.

    Conclusions:

    • The study indicates that auto-antibody-defined subgroups of rheumatoid arthritis may exhibit genetic heterogeneity concerning Properdin factor B (Bf).
    • The Bf SS phenotype is confirmed as a potential marker for rheumatoid arthritis susceptibility and/or disease severity in specific patient populations.
